It doesn't seem to be a common occurrence for sure. Whether that's because it was anomalous or just doesn't occur over population, I don't know. Sometimes debris is deliberately sent tumbling to help more completely burn up (space shuttle ET, Skylab, etc.) The flight has been called a success FWIW.

Many instances have also been recorded for similar trajectories

They also do happen over land since the burn is targeted to dispose the vehicle in the Indian Ocean
