Agree re urgency, with the perspective that we do most of our own biopsies. Also slightly influenced by transplant versus native. I worry much less about aspirin in a transplant biopsy than in a native where there is really no effective way to apply pressure. #Nephsky
I never looked it up 😬 just an empiric approach that worked so far (ratio: the transfused platelets are not affected by ASS and should work to close the puncured vessels if transfused peri BX)
There are good data from Scotland suggesting that it is safe to do biopsies on aspirin and there are also data to suggest that holding aspirin for 7 days prior to a procedure may be harmful. I think there is enough equipoise to suggest that we need a trial but in the meantime I don't hold if urgent.
SRR Biopsy register: We now have 10 years national registry data presented by Prof Colin Geddes at SRA 2024, write up pending: no difference in bleeding rates on/off aspirin.

Generally RPGN, nephrotic syndrome with acute complications and allograft AKI with strong suspicion of rejection I’d say are urgent
