I am loving the idea of a Pig Powered Single Person BattleWagon.

It frees the hands to use Bow & Arrow, Shield & Sword, or Spear.

The Pig would respond to standard verbal commands, like sled dogs-

“Cry Havoc and Let slip the Pigs Of War!”

I reckon you probably slather it in pine tar for grip, and have studs sticking out to increase traction. Gonna need a real good cushion, because without rubber, you need something working as a shock absorber

If we are talking medieval, I recommend horse drawn? For an off-road wheel chair, you might consider a dog or dog team. Wide sled style makes sense.

My 85-year old mother grew up on the prairies and they used a horse drawn sleigh to get to school in the winter.

Depending on the style of fantasy this may not be a top consideration, but a huge limiting factor is that a wheelchair made of medieval materials on medieval roads is going to be absolute murder on your shoulders, especially if you have a higher injury level.

That checks-out. The kid of the dude who invented tires was getting headaches from riding a tricycle on the cobbled streets of Belfast. I think the earliest attempts at tires were leather. In a DnD context, suggest some magical creature’s leather, could also make building the chair a resource thing.

If the setting supports it then I’d give the chair legs. If realism is a big concern and this person needs to fight then I think you’re stuck with either a cart or a litter with bearers. Barring any of those, then I’d look at a modern wooden wheelchair and put a medieval texture on it.

Ok, here's what I was thinking of. It's not really a medieval aesthetic, but a deliberately simple, modern DIY design with replaceable parts and attachments might help. Their trike designs in particular looks like something that I could imagine on a medieval fantasy street.

I don't think it's really possible to make a rigorously realistic all-purpose dungeoneering combat wheelchair, so I wouldn't worry about it too much. Modern sports wheelchairs are pretty specialized: hiking, fencing, and tennis chairs all look different b/c they're prioritizing different things.
