There's a big gray area (below clear toxic behavior) when stories clash or minor issues aren't reported, people are contrite, you want to help resolve things and not "give up" on people, etc..
Good intentions & common sense aren't enough, imo
OK, the main text is "Building Successful Online Communities: Evidence-Based Social Design" by Kraut and Resnick (2014). Ch 4 and 5 specifically deal with regulation of behavior and welcoming newcomers. It's about 100 pages for both chapters.

We’re saying, ESPECIALLY if you’re a dude in an org, LISTEN TO THE WOMEN who tell you “that dude’s a creep.” DON’T defend him “because he’s fine with me.” Get rid of him!
I’m arguing that they didn’t TRY to get rid of him, because they didn’t consider the complaints against him as having merit
