My old friend Michael once ran an encounter where we fought a giant animated tomato who revved himself up and then shot back and forth around the arena like Sonic the Hedgehog, so that takes the cake easy.

As a DM, I tend to throw other parties at the players!

There are no hard rules here, but my personal opinion is that no amount of fictional horror and misery is definitionally too much. A few spots of genuine beauty can really make even the most brutal story pop.
