Basically there are 3 different stop points: Not filling prescriptions, not reaching a therapeutic dose, and stopping despite obesity being a chronic conditions and these being treatments, not antibiotics, for weight /2
Not filling likely in part due to prescriber attitudes and in part due to cost and coverage, not reaching therapeutic dose likely due in part to side effect and again in part due to insufficient prescriber follow up (to help for instance mitigate side effects) /3
If you've been battling obesity for decades, there's programming in your brain that says, "everything would be fine if you just lost those x pounds." Sadly, it's tough to accept that your normal is not normal for those around you.
