Shoulder pads are making the shoulders of the jacket hold shape but the sleeve of the jacket is too tight on her upper arms, so you get that weird little indent between where the sleeve drops off the shoulder pad and where the fabric of the sleeve is snug.

tl;dr: bad tailoring.
