For all the headlines from todays SDR, this is HMS Audacious at Devonport in July last year. Since then they've finally moved into dock, but this is the second newest Astute and it was last operational in April 2023... Without solving todays problems, talk of upto 12 is just fantasy fleet building.

I think all seafarers can already take an educated guess on the cause of this accident. The ship had 16 crew, which almost certainly equals the OOW working 6on/6off with the accumulating and crippling fatigue that comes with it. 1./ www.bbc.com/news/article...
