The stated reason is that the Texas MMRC had a backlog on top of the two-year lag typical of such committees. But the person who said that also recently appointed an anti-abortion doctor (Ingrid Skop) to one of the committee slots

This behavior isn’t limited to Texas. After ProPublica reported that Georgia’s maternal death committee determined two women died preventable deaths from being denied abortions, state officials responded by firing all of its 32 members for breaching confidentiality.
