the first time i went down the stairs in the historical society in silent hill 2 and realized that they were impossibly long. also, everything that follows in the prison, lol.
I love Toluca Prison, there's a locked cell that occasionally makes strange noises. If you aim your weapon James will lock onto something in the cell but we the viewer are unable to see it and bullets do nothing 👀
You can actually kill that "thing" if you aim carefully (it screams like a normal human), but we won't see what is exactly. And the "ritual..." word that murmurs is really creepy.
Besides of P.T., maybe the best horror experience was with Eternal Darkness on GC. That bathtub scene...
It's not a specific moment but I'm not sure I've ever had a game nitrous-boost my heart-rate than the medical ward section of Alien Isolation. That first time playing and experiencing the moment the Xenomorph becomes a real threat will never leave me
I don't play too many horror games, but Resident Evil Village, the doll house for the first time had me on edge. Going from guns blazing to everything taken and I'm now in Amnesia/Outlast was terrifying. Especially the build up with the stillness and PT inspiration was insane.
It was in the original Fatal Frame, I found out not all ghost appearances were scripted. I was backtracking, not expecting anything. Running through the different rooms trying to find progress. Then, at the multiple screen doors the ghost detector suddenly lit up. I actually dropped my controller.
Not sure if scariest, but (trying not to spoil) there's a moment in Sally Face where everything you've experienced has led you to believe that what you're doing isn't just the correct decision, it was the righteous decision. It was unbearable not just watching, but personally performing the actions
A weird choice, but playing the nighttime Terrorist Hunt maps alone in Tom Clancy's Rainbow Six Vegas 2 can be surprisingly tense. The jumpscares are also insane and it's not even trying to be a horror game!
Not a game but Planetarium Gold - clicking on a black hole or learning about the future of the universe (heat death) would cause a tombstone and eerie music to pop up on the screen. Kickstarted deep existential dread in an eight year old.
FNAF in VR, I've never been so scared in my life. Other than that, the worst concept for me is seeing something like a face staring at you in the distance/dark, which is why that exact scene of Lisa just looking at you from upstairs really gets to me as well.
The one I can remember is one of my first regular in-game nights in Darkwood. The unknown you don't understand mixed with not knowing the amount of danger you're in is the peak of horror for me, and that game blows any other out of the water in that regard while you are still learning
In Silent Hill 1, the school section kept me on edge. I think the combination of the soundtrack and the impossible layout with the bathrooms being elevators gave me a good deal of anxiety while playing it. Like nothing made sense, and the game was messing with you. It's great!
The Cynthia ghost fight in Silent Hill 4. First there was the cutscene in which she crawled through the subway turnstile, excruciatingly slowly...and then gameplay started, and I was NOT prepared for how quickly she HURLED herself at me. I turned the game off and didn't touch it for like a week. XD;

In Killing Floor, the Patriarch is a Zed that can go invisible, he pops out of nowhere, hits your ass, sends you flying all whilst you are still screaming from the jumpscare.👍
that was so good and in a time where anything dynamic in pre-rendered games were easily identifiable because they stuck out so much against the backgrounds.
Playing Dead Space for the first time, there's a corridor before the tram repair bay in chapter one where the lights go off and the ambient noises pitch up. I had to pause the game and invite a friend for a sleepover before I could continue.

There is a stairway with a window in Amnesia Rebirth. As I descended, I saw a faint shape against the barely darker backdrop of the corridor below, moving up and down, just up and down, like breathing. My skin crawled for a moment and I had to pause. Scared of my own shadow.
