Common sense again from Ed who just ‘gets’ this. The NHS and social care are totally entwined. Planning
and investment needs to be integrated at a place level. Health funding without social care funding will leave fundamental issues unresolved and many people suffering

2 years ago my mother in hospital, ready to go but needed a lot of social care (which being rich she’d have to pay for)
Discharge nurse gave me a handwritten list of suppliers. There should have been a database of spaces

Problem was she lived long way from me, and my main communication was with hospital (who were great)
There were no suppliers (she needed two visits a day) given her being in ‘remote’ village in (North) West Sussex
Eventually got her into a residential home for ‘convalescence’ (then permanent)

Fair enough. Sounds like a failing of the hospital as they could have referred her to adult social care before discharge. As ever though, if you dont know this stuff you wouldn't know to ask. Social care information should be given at an appropriate time after admission so everyone is informed.

I resented that she got to stay for free in hospital, but pay (a lot) once out because she had savings (on which she and her husband had paid tax), but then when she died there was a fortune to pay in inheritance tax
