So, I looked it up, and Shadowheart always choosing to kill her parents despite you fulfilling certain criteria was a bug that was fixed in #BG3Patch8! This video explains the bug in more detail.

Oooh I remember watching the Gale Mystra points video by the same person. It's really interesting how many hidden point systems the game has, and as a programmer how easy a single boolean check the wrong way round can break things 😆
