Could be one of the upsides of an autonomous vehicle fleet, if/when that happens (though I'm not an AV booster); society simply wouldn't accept the operators - waymo etc - not having a safety culture akin to that of the airlines (and they'd be mad not to have)

I was a commercial pilot in Australia, and we had a mechanism that allowed reporting of near misses/mistakes. This is a government-funded body that was created to improve safety. We need something similar on the roads. All pilots make mistakes. We have more than one layer to mitigate any one error.

Where I live, when there is a serious RTC the police investigate if any one was at fault & the local council investigates if the road layout was at fault or could be improved. Improvements are added to the roadworks schedule.

Perhaps what is not needed is some commission at a gov’t level as this allows for road safety to become political issue, e.g. one government sets maximum speed limit at safe but lower level & the next gov’t raises it but only on some ‘faster’ roads. 🤷‍♂️
