It was called Jak & Daxter: The Precursor Legacy - The Lost Levels, a Flash game released in 2001, and I have the files archived. :) It has been uploaded to the Flashpoint Archive project, and can also be downloaded individually from here: https://jakanddaxterarchive.home.blog/downloads/
