Do computerized diagnostic interviews could? Cuz the CIDI has a self-report version. Or you looking for more standard continuous score measures?

Sharing out of interest but not exactly what u asked for: https://www.jstor.org/stable/351672 A Guttman style scale where ppl need to check off certain # of items at level 1 to move to level 2 and so on.

Hm cannot think of anything that would resemble this (eg DSM5 ptsd criteria where you need a few ticks per sub scale to qualify)

Oh perhaps the AUDIT ? I think you may have to tick some items in categories (eg legal trouble, craving, withdrawal) for a putative diagnosis.
