Has there been a dungeon TTRPG that used a map and minis (or standees / flats / counters) and actually came with them, in a box, as a pullout, etc.? Starter sets are appreciated but don't count.

Yeah, that's more of a starter set, but definitely what I'm thinking of. It's interesting that "starter sets" get that kind of full treatment while the "main" game doesn't.
A lot of adventures did come with some components, I definitely thought of how a later 4e monster guide came with tokens- but I'm thinking chiefly of core games.

I don't know why street level supers games are always hitting that paper minis note, when it feels like they would need paper minis the least, but its fun that they are
On a more dungeonous note MAYBE WHFP 3e is closer to what you're thinking of, at this point I have no idea what's even in this box outside of every component imaginable and "every component imaginable" includes maps and minis too probably
The Dragonbane core set, while it looks like a starter set, is actually the full rules (including a substantial bestiary), an entire campaign, standees (NPCs, PCs, Monsters!), dice set, reversible battle mat and world map. It's a fully playable box, literally everything you need.
