Most rural areas are Republican leaning. This will affect the people who vote for them. When/if this takes affect, come elections, I expect a political shift.

Medicaid is the largest source of funding for nursing homes in the United States. Are they going to be rolling Grandma and Uncle Joe to the curb in their wheelchairs? Of course they will say states, who often can't legally operate in the red, can take over the federal funding. They can't and won't.
