At this moment in 2003, the 7-member crew of the Space Shuttle Columbia died when the orbiter began to disintegrate over Texas and Louisiana during re-entry due to damage to the leading edge of the left wing caused by a piece of foam falling off the external fuel tank at launch 16 days earlier.
