In a vanilla playthrough, the only thing the affinity mechanic affects is which character is standing beside Django in the epilogue. Eligible characters here being Lita, Violet, Lady, Zazie, and Sabata.

However, if you took a JP copy of Boktai 2 to a Nintendo Joy Spot during the month of February, the character Django has the highest affinity with will give him a valentines day chocolate.

While the event was Japan only, the English ROM still contains the event itself. If you want to try it out yourself + other Nintendo download station exclusive events, you can run the following Lua commands after loading your save on an emulator. (Credit to Raphi for finding this out)
