They were built through a LOT of trial and error. Boeing looked into trying that as they (due to mergers) were part of the building process for Apollo. They scrapped that as it was built with processes not taught to engineers today.

The Saturn 5 was built using blueprints and slide rules and never once blew up. The Musk Starship is intrinsically flawed and will always have a huge probability for catastrophic failure due to high number of engines.
