Importantly, stroke and death occurred even when arterial trauma/cannulation was immediately recognised, demonstrating the need for robust and objective checks before dilatation to prevent arterial trauma in the first place.

You'll have a better chance if you've seen them before. Suspect any line which doesn't follow the normal course (tip in lower half of right mediastinum, pointing caudally; L line crosses midline, R doesn't). But it's basically the wrong test for this question; confirm a venous pressure waveform!

This. And if your unit/department doesn't have a culture or policy of transducing lines to confirm placement, it's just waiting for a (belatedly recognised) disaster to happen before making the change - as has happened at many hospitals up and down the country.
