It looks dry and scaly to me. That might indicate allergies. Discuss with your vet.
With allergies, is worth it to also consider asthma. Any changes in breathing? Wheezing? Increased hairballs, or behavior that seems like hairballs that don't come up?

he also started new meds and a couple months later changed the source, so maybe allergies to an inactive ingredient...
Most animal allergies are protein sources in their food. They can develop allergies at any time, so it can be in something he's been eating for years. Is he grooming more?
See if you can eliminate possible injury. If that's a scrape, I'm willing to bet there's evidence.

guessing he clipped himself yeah. will keep looking for a suspicious tuft of furt & keep it clean
we're under a few inches of snow and it's like 2 degrees so not rushing to the vet but will get there if it gets worrisome
