I just had a great few weeks in Japan. Among (many) revelations, I was inspired by the through-running regional trains onto metro lines (eg, Odakyu Odawara regional line onto the Tokyo Metro Chiyoda Line). I thought it was going to be complicated, but it's self-intuitive & good for riders.

But this would be complicated, because of #1 the need to electrify the regional trains; #2 the need to buy new trains; #3 different FRA & FTA requirements.

But if this were Japan, there would be both S-Bahn & Metro services.
Do any of the Tokyo/Toei Metro lines go further out from Tokyo/Otemachi Station than even 20 km?
But more to the point, I don't think DC is ready to hire people to push them onto the Metro trains the way Tokyo does.
You especially don't want to mix them if the regional rail shares track in any way that it could be delayed at all completely messing the metro
I used to live in Yokohama. While the train system worked marvelously, I never cared for the morning crush between the ‘hama and various stations getting to the Yamanote Loop.
