Like you ask user to input a single digit number and he enters (by mistake or maliciously) a very long string, crashing the system or even giving him unauthorized access (hacking).
I immediately thought of: you’re a designer of a critical public service and you ignore the accessibility needs of a minority of the population. (Think less colour contrast & font size for visually impaired ppl, more cognitive impairment). Which renders the critical service unusable for them.
Basically it's a term used to describe when a person tries to do something that most other people would never do, usually because of a technical or behavioural reason. eg. '99.9% of our users are on iPhones. The 0.01% trying it out on Android are edge cases we won't worry about'.
