Austerity policies started the decline. Look at the graphs for food bank use. The same kind of steep increases are visible post 2010.

Steve I am in a distinct minority having managed a large NHS dept and a moderate sized private healthcare business. Most HC managers only have significant experience of one sector. Through this experience I have knowledge of what is good and bad in each. NHS should make more use of pvt provision.

Our NHS workers were not adequately protected during Covid. How many died, how many quit, burnt out with MH issues, how many fled to other Countries for better pay, access to accommodation, better working conditions or a better life.

I qualified in 2005- they seem like halcyon days now

Huge investment, prompt out patient appts and procedures, easier GP access, lower ED waits, a happier workforce (we actually had a study budget) & a healthier population - now we have reversing life expectancies & shrinking children...

2004-2011 the best period of my 30yrs a GP,
Patient care was prioritised and staff on the whole were treated well.

Agreed- joined my VTS in 2007 and got my CCT in 2010.

The sheer numbers of people leaving hospital medicine to join primary care training was huge and GP was the most competitive specialty.

Things actually worked and patient were actually seen and treated in secondary care.

NHS waiting lists fell
GP numbers were rising

But there was a cost - UK had to join the rest of developed world and spend on health proportionate to GDP

DIY or hand IT over to the UK private sector so that they can sort IT

There are lots of things wrong with the NHS but it’s the way the UK funds its health and this isn’t too different from other nations in that it’s a national government directive

Politicians meddling: Since 1970 politicians have "reorganised" the NHS on average every 3 and a half years. From RHAs, DHA, CHCs to Bottomley's "internal market" and Lansley's disaster. With people re-applying for jobs each time, it was all such a waste of money that could have been better spent.

In the early 2000s it worked well, waiting lists fell, plenty of GPs, decent infrastructure. No extra money has been put in since and buildings left to deteriorate along with staff morale.

Metaphorically speaking, the NHS shredded its corporate memory in 2010 to save on storage costs for all the document storage. In some areas it still hasn't remembered what it knew then.

I'm also convinced there is a fundamental failure on the part of things like NICE using a cost per QALY. Because the principle is that if you give me back a year of perfect QoL I will contribute to the state. Or at least my family won't stop contributing etc.

That means that those are other costs. If I have to take a year out to care for my relative who is unwell... You have costs of care for relative etc. but you have loss of tax from me. All sorts of tax... Income, NI, VAT, Fuel Duty, Airport etc. I buy less goods so the shops need less staff..
