My agency is just me (and two rabbits who refuse to do any reading), so I review every query (though I am exceptionally slow!).
The first thing that I notice is the title - ideally it will catch my eye. Then I skim the query, and jump to the body of the text. So it's a little bit of everything!

I'd love to hear about your process for reviewing all those queries. Do you glance at each one yourself, or does an assistant make the first run-through? For picture books, do you look at the query letter first, or skip straight to the manuscript? Thanks for answering our questions!
