He's an asshole, but if you're honest with him he'll often downgrade it to general snark. It's when you lie about health issues or things you did leading up to the problem that really brings it out of him.

According to my mom, Scrubs, while exaggerated, was the most accurate depiction of a hospital on TV. She said she knew someone like every character on the show, and the janitorial staff knows *EVERYTHING* so you treat them right and with respect
