Black holes are messy eaters. Usually (at least it's hypothesized), the matter stream ejected comes from part of what it is eating. As for black holes at the center of galaxies, most of the galaxies observed do have them I believe.

It's my understanding that there's a black hole in the middle of all galaxies, and what's even more fascinating, the size of the hole corresponds to the size of the galaxy spinning around it.
