As a prostate cancer survivor, and considering the level of health care he’s received for decades, I’m really surprised that this wasn’t caught sooner. A Gleason score of 9 is really bad and I didn’t think this could get to this point just overnight.

well, the sufferer has to alert the Drs. to symptoms, which can pop up at any time, BEFORE a Dr. knows to look into it. Drs. don't typically "notice" things, unless they're looking at imaging or blood testing etc. already done in that body area. And they can miss seeing stuff. Science
