Chris - without checking google or the literature, I could say that there is an extant population of Arctic charr in the Lac des Neuf Couleurs in the french Alps at 2874 m. Cheers!

For Sweden, the highest-altitude data we have in our electrofishing database is 1158 m (coord: 62.974681, 13.036263). That is a brown trout population, self-reproducing. Such high-altitude populations could be stocked though...

Data exist from 1997 to 2022 from this site, so I would say it is self-sustaining.

Rocky Mountain Cutthroat Trout (new AFS name) are in several high elevation lakes in Colorado's San Juan mountains. Most of these require stocking, but at least one or two may be self-sustaining populations. I think the highest of these is ~11,700' in elevation.

Rio Grande cutthroat trout in New Mexico are at 10,000 or so feet if I'm remembering correctly? Southern end of range intuitively makes sense for highest elevation populations.
