In practice, this would require directly taxing loans over a certain amount (say, $150 million) and/or requiring a matching amount to be deposited into a housing fund or else directly given to Habitat For Humanity or something.

I would say you can't amass that level of wealth. For those who already have it, their wealth goes to the cap, and everything leftover automatically takes care of things like housing, food, healthcare, and environmental protections. Otherwise, the cap is 5 mil. Period. No offshore accounts allowed.
