This is Compaq's "Business Wavetable Sound Card" that just so happens to be a great card for DOS gaming 😜
It doesn't look like much, but it has the ES692S wavetable chip on it (1MB of samples) and the very capable ES1869F chip for digital audio and FM synthesis.
Great all-around card.

I got a similar card with Admos QDSP700 instead of the ES692S synthesizer, and it's perfect for smallest footprint systems, since it'll fit any system that even has a free ISA slot, and brings so much to the table.
