The difference between cis HRT and trans HRT reminds of the difference between getting an elective vasectomy vs. an elective hysterectomy. The former is met with little resistance, offered on informed consent; the latter is a last resort, met with a thousand are-you-sures or refused outright

It's amazing how quick a doc will give you the option of gender-affirming care if you're cis.

That said, it was still far less in the way of barriers than my first wife faced for sterilization and what I experienced for medical transition.
Granted, once she found one who believed her, the process was simple.
There is a (good) belief out there that sex per se doesn't matter
(in business, in school, in the military, in love)
But it does matter, in one crucial exception
(in the person's identity)
Most don't get the exception.
