When I got masculinizing top surgery I had to get a psych letter. It was coded as a double mastectomy. If I’d gotten breast reduction no psych letter would be necessary.
For what it’s worth, I’m a cis woman who desperately needs a breast reduction as my naturally large balloons destroy my shoulders and have caused me decades of pain, I don’t get approved either. I’ve had two spinal surgeries instead.
