Question to my transplant nephro followers: after recovery from PJP pneumonia and years of prophylactic dose septrin is it safe to stop or would you continue septrin prophylaxis forever π€

My patient is years after PJP and still on prophylaxis. My respiratory colleagues claim that this situation warrants prophylaxis dose septrin for life. Dont think I have ever done that π€
It looks like βnever change a winning horseβ, and if nothing happened for so many years on TMP-SMX, itβs hard to guess what will be after its discontinuation π§ but data about long-term prophylaxis almost do not exist π€·ββοΈ
We used to do Bactrim lifelong after kidney txp in all pts until a few years ago but now stop after a year. But for someone who has had PJP, I would prefer to continue lifelong.

But in this study prophylaxis was used for 12 months and most PJP cases were diagnosed after 4y
Until a few years ago we did SMX/TMP for a year only
Then we had an outbreak (preCOVID) irrespective of transplant vintage - and now we do it for life!
