My PCP suddenly has no vaccination appointments until April. I'm going to Target this weekend to get my MMR started because my titers came back non-immune and I can't just get a booster (in school and applying for health sciences, two vaccines is required).

The tl;dr 1-post summary: Get your MMR titers checked or a booster shot if you were born 1957-1989. Get a Tdap booster if it's been more than 5 years since your last one. (The CDC says 10 years; the CDC is wrong.)

I and many of my friends were born 1952-56. I'm surprised how many *don't* recall getting measles--I did, and boy howdy it is NOT something you forget. So if you're in that slightly older group, you should definitely check, as well.

It used to be 10 years but there's evidence that immunity is starting to wear off faster (with a couple of theories as to why, but no hard evidence yet)

IT'S COMPLICATED. Basically: the whole-cell pertussis vaccine used until the 90s gave pretty good immunity (and made the disease less severe if you got it in adulthood after the childhood prime immunity window wore off). The new acellular vaccine is much less long lasting.

Its protection against severe disease is also less good. We switched because of, you guessed it, antivax campaigns about the side effects. Studies have been done to show the acellular pertussis component drops below 50% VE at ~4.5 years after last dose and hits functionally 0 around 8.

The only concession to this basic fact has been the CDC switching from "1 dose Tdap in adulthood, then Td every 10 years after" to "1 dose Tdap in adulthood, then Td or Tdap every 10 years after".

The increase in pertussis outbreaks means most providers quietly switched to only using the Tdap, not the Td, for the 10-year booster, but for your own protection, every 5 years is better.

Pre 1957 you're presumptively immune from childhood exposure; the end date is because they added a second dose at 2 years old in 1989 and a lot of people didn't get caught up (in some cases colleges required the second dose for enrollment, but not all of them did)

1957-1965 or so also got the first generation measles vaccine, which was made with inactivated virus instead of the current live attenuated virus, which wears off over time, so it's extra urgent for them to get boosted.

Tried to check MMR titers last spring, my doc wasn't as motivated as I was. Work is sending me to Dallas next month, so I did a walk-in booster at CVS. The internet was unhelpful - do you know of risks I should have considered, or was "better safe than sorry" a valid strategy for a mid-40s person?

There's a very very small chance of neurological complications from any vaccine, but it's extremely rare (for MMR I think it's around 1 in 4 million doses when you control for the baseline chance of developing the symptoms normally). It's not anything I worry about, personally.

Weirdly, my measles & rubella titers are fine. But mumps antibodies were nonexistent, as was varicella, sigh. I wish I could boost MMR again & get the varicella series, but doctors said immune med & live vax do not mix. I did get Tdap & the latest pneumonia, due to boost Covid next.

I did some gentle harassment and I got the hubs to catch up on his. I got my pneumonia yesterday and am waiting on the blood work for the others. I will probably do a tdap even tho the doc disagrees (I'm at 8 years)
