It may have been Keith Parkinson, who did the art for Darkwalker on Moonshae, the first book to feature it; the lettering loosely resembles his handwriting. But I don't know of any official confirmation.
Making some assumptions - could be Stephanie Tabat; as they were graphic designer for a lot of the lines back then (Spelljammer, Forgotten Realms, etc) but she is not mentioned after the AD&D1e version.
It is clearly independent of the cover artists. Likely not typographer Betty Elmore.
It is a bit of a shame that it isn't more overtly stated, and that most of the artists who could've done it don't have it listed/have portfolios online due to how old this would be as a design.
But it'd be nice to see some Arnell Group type utter madness about the design process.
