To me it looks like a cleverly masked and morphed hi-res fabric texture which the shading and smaller details like folds and strings are then layered on top of. I could be off base though, only they know for sure.
Is the fabric texture a brush? If it is, how did you stitch (heh) it together with the tears/runs so perfectly? Also, there's no way you curved every individual thread of that fabric to match her contours... right??
Also also, is she wearing those pants backwards???
texture image I cobbled together (the tears and runs are honestly pretty simple to make, just follow along the shape and draw them, they're not part of the texture, rather, drawn over)
Also, liquify tool is your friend, used it for the curvature!
