The role of intermediaries prior to seeing a qualified Dr is dangerous and breaches the founding principle of the NHS, equity in accessing a Dr.
It took decades to build trust in the NHS against years of hidden treatable diseases. A return to those days are likely. Late presentation = more expense.

The PA's I've met all have a vocational MSc and a relevant 1st degree. Speaking as a nurse, I see no reason why they shouldn't be doing the menial/routine medical tasks, because nurses usually have better things to do.

Interesting. In the US there is trouble attracting new docs to primary care and in Canada there are 10 000's of people without family docs. I think the thought is more stop gap. In the US I bet saving money is the biggest driver. Not that they say that.

Primary care doctors here mostly refer you to some specialist if the issue you are having goes beyond the basic antibiotic or other treatment.

In the US people seem to want to skip their PCP and go straight to a specialist. In Canada the pcp tries to avoid the specialists. It keeps costs down.

That and, until my current one, every PCP I’ve had has been useless for any kind of complex problem … if you see them ar all and not an NP, or for more than 30 seconds. Then wait while they run the same tests, shrug, and send you off.

If I know I need a specialist, it’s faster to skip all that.

Maybe. I don't have a copay but that is a good point. In Canada it is all one system. It costs 3x (I'm guessing) the amount to see a specialist raising the cost for everyone. There are a finite number of healthcare dollars. They are used wisely.

I found myself with a PA recently. He took some blood, like nurses do. He said being a PA was a faster route into healthcare than Doctor, but it’s a role with zero career progression. I found the whole encounter rather unsettling. A kind of human robot with limited programming.

My primary care person is a PA.

She mostly just does the basics of any "well woman" exam like drawing blood, taking BP, etc.

Anything else, she refers you to a specialist.
