why are you requesting legal full name of applicants before they’re accepted? you’re going to have a google docs spreadsheet of a tremendous amount of PII. why add to the risk?

You should probably specify in the form if by NDA you mean non disclosure agreement or non disparagement agreement (or both). They both have the same acronyms but mean very different things.
