Pro tip not mentioned enough: When I fist top and feel tension, I DON’T pull on it. Instead I stop at the point of tension. Like a tense muscle, it doesn’t need movement. It needs time to adjust and relax. Stay there. DON’T MOVE. Feel it slowly relax. Very slow, very tiny rocking motions help.

I use the Carlton Butt Clock a lot; it's based on this same methodology, but using quadrants of the internal sphincter. I find it awesomely effective.
