One reason this happens is because we're simply not a serious country & don't hold our leaders accountable. The other, however, is institutional. There are no mechanisms aside from impeachment or fixed elections for no confidence votes in the chief executive.

One of the major flaws in the Constitution is there is no way to call a no-confidence vote similar to many parliamentary democracies. Fuck up badly enough and the opposition can bring down the government - usually with the help of backstabbers from the governing party. We are just paralyzed.
