I can't speak to the Horn of Africa specifically, but there's a ton of suspicion and outright rejection of "modern medicine" throughout Africa generally.
Certainly it's a perennial problem when responding to Ebola outbreaks.
Yeah my eyebrows also raised a bit at that generalization. That said, I have watched people line up for hours in Angola to get the shots of AstraZeneca vaccine that Canada decided was not usable in Canada. (Not saying it was bad to be anti-AZ but privilege does allow you to be pickier about care.)
