If the shelter doesn't quarantine new arrivals until they are sure, it can happen. A decade ago the old overcrowded Irving Texas animal shelter unknowingly put an infected dog in the general population and had to destroy every dog in there. TG their new shelter has adequate quarantine facilities.

Other dogs that were dumped and carried it. I doubt the puppies were vaccinated by the people who dumped them. The small amount of time that the puppies were brought in and exposed before being vaccinated themselves is more than enough.
