Lady Death used to be the only reaper in Noctlem, but after a decision was passed to reincarnate one of the angels, she decided she could not make these decisions herself and created a second reaper, who then.... Became too anxious to actually help her make tough decisions.

So these two are under the employ of the angel of the grave, who is basically an overworked Hades figure who decides where souls go when they die- be it to his or his sibling's divine pools, or to other gods in the world, or into limbo. Lady Death was his only employee for a very long time.

Then came a big of an incident, involving another angel allegedly going mad and the angels having to decide if they would, for the first time, reincarnate their sibling's soul to give him a fresh start. Which would completely erase the person who came before him.

This decision ultimately fell to Lady Death, for who the angel was like a son she followed on the battle field. The two had a very good relationship. And in the end, she could not make the decision to reincarnate someone like this and never wanted to make the decision ever again.
