My insurance company, Harvard Pilgrim, did this to me. I had breast implants that had incapsulated (formed scar tissue) that was becoming painful) on both sides. My insurance approved one side only, even though both were affected.

The doctor’s assistant told me I had to pay on the way the OR, chasing me inside with a credit card machine. I was pretty upset, but paid. I appealed. Lost. Appealed. Lost. Appealed to an outside panel, and won. Assholes. I NEVER go away.
