The problem with this analysis is that it isn't safety Rocket Boy hates, it's regulations. He blows up rockets on purpose to learn, but he's not going to get on one himself until he's got the bugs worked out.
I suspect that Musk would actually pass on the opportunity to go to Mars when he finds out there is no way to strap his offspring to his body as a form of life support and shielding against cosmic radiation.
I mostly hate that "the worst people in the world are obsessed with going to space" is giving "going to space" a bad name (and also currently defunding NASA)
Imagine being told you are the smartest person in the room since birth (when really you can do calculus about as good as some syphilitic idiot from the 1800s) and just assuming from there on out that every thought you have is divine wisdom you are imparting on humanity
Give it time. Some overworked engineer might end up making a massively critical mistake when the Apartheid Emerald Mine Heir finally decides he wants to ride one of his rockets.
I for one think we should prioritize the tech required to get Elon Musk to Mars, just without regard to the tech required to bring him back, nor to keep him alive on the Martian surface.

Should he be inside the rocket, or strapped to the outside of it?

No one seems to ask why musk has never once set foot in any of the roman candles he sends up.
Let alone be in it when it's lit.
For someone who wants to go to Mars I say "hurry the fuck up with it."
