Honestly, I'd say only they notice 9 times out of 10, but there is some fun to be had with 1-2 folks with them noticing, or maybe being the one causing it
Since I’m observing from outside their universe, I prefer no one noticing because it feels so excitingly corruptive. No one can even fix the change anymore because it feels, and is, normal. I especially like when unaware changes occur due to an accident — wish gone wrong, warp device mishap, etc.
Specifically, I like when there's only one person who notices, whether that be the victim or a friend of the victim, trying to convince them that something's changed 👀❤️✨
I'm on the side of only the victim. At least at first. But then steadily the warp in reality effects their thoughts and things become normal for them too.

I get to view pictures and stories from an omniscient third person perspective, making it impossible to fully have "no one at all" notice.
The sooner my mind gets to see the aftermath, that the world just kept going along and continued just fine, the better for me-

There's often a "how could they enjoy THAT" dismissive attitude though that's only looked at it from one perspective. Annoying when that happens
And live happily in their new reality and form
And yeah... I worry about reality alteration becoming the word Permanent 2.0. A horny tfy buzzword xD
For the former, it plays into that 'new body' side of TF that can be fun. Figuring out how to move, these new sensations.
With the latter it playa with those who like altered perceptions, the idea of something strange happening while being unaware
