Calls to mind the old Thieves World series of shared universe short stories. They started out with explicit rules that you could have other authors' characters in you stories, but you couldn't inflict life altering changes to them.
After spending a week helping a player craft their character for a Warhammer 40k TTRPG, during their first session they tried to motivate a group of civilians to follow them into a warzone by shooting one of the group, not understanding that the rest of the armed militia would take offense at this.
Hahahahhahaa omg, this is such an amazing first move. So did you give them a mulligan or did they make a new character or was their first session also their last?
They had a backup character waiting to go, since the PC mortality rates in the Warhammer 40k TTRPGs tend to be quite high.
In our last session a PC techpriest recoiled after plugging themselves into a terminal, and one of the other PCs overreacted and shot them in the back of the head.
To be fair though, in a previous session an NPC tech plugged themselves into an unknown system and got infected with a computer virus that turned them against the party.
They caused three critical wounds that needed hospital care before they were stopped, so there was good reason for the reaction.

The whole party died from exposure to a dangerous chemical.
He did not let us play as ghost, he did not let us mulligan, we didn't even get rolls to see if some of us survived, just instant death.
Nobody was happy, not even him.
We were pissed because he killed us at our first action.
He was pissed with us because "we didn't see the warnings".
The "warnings" were a piece of paper in English, which at the time non of the player knew how to read.
Some of our characters were supposed to know English.
But we didn't get a translation of the paper in French, just a "figure it out".
It's been more than 20 years now and I'm still sad about it.
