Ask them for the name of the doctor, specialty, and license # who denied coverage.
They will back down.

Definitely file an appeal. I know Humana kept denying certain coverage for my elderly mother and I would file an appeal. I won them and yes it was multiple denials. It takes a little time but worth. And if they should deny that you have a right appeal that second denial. Good luck.
